Paid Coaches

All paid coaches employed by the Moon Area School District must complete the following clearances, forms and training requirements before being considered for employment.

ACT 114 – FBI Criminal History Clearance

The Commonwealth of Pennsylvania has changed the provider for this clearance. For additional information regarding the process, click here.

To register for fingerprinting:

1. Visit https://uenroll.identogo.com/
2. Enter the service code 1KG6XN
3. Select Schedule or Manage Appointment.
4. Complete all required information.
5. Select a fingerprinting location, date and time. Walk-in availability may vary by location.
6. Pay the $22.60 fee at the fingerprinting site.
7. Follow the instructions provided after fingerprinting to access your results.

Important: Results are not automatically sent to the school district. Applicants must provide the district with the UEID number shown on the receipt issued after fingerprinting so the district can access the report.

ACT 34 – Pennsylvania State Police Criminal Record Check

Apply online at https://epatch.state.pa.us

• Select Employment as the reason for the request.
• Save or print a copy of your completed clearance.
• Submit your results to the Athletic Director.

ACT 151 – Pennsylvania Child Abuse History Clearance

Apply online at https://www.compass.state.pa.us/cwis/public/home

• Create an individual account.
• Under application purpose, select School Employee Governed by Public School Code.
• Save or print a copy of your completed clearance.
• Submit your results to the Athletic Director.

Required Forms and Training

Paid coaches must also complete the following:

• ACT 126 Mandated Reporter Training
• ACT 24 Arrest/Conviction Report and Certification Form
• ACT 168 Sexual Misconduct/Abuse Disclosure Form

Tuberculosis Test

Pennsylvania Public School Code, Section 1418(b), requires certain school personnel to be tested for tuberculosis.

This requirement applies to employees and independent contractors who provide direct services to students. The Pennsylvania Department of Health defines direct services as regular direct contact with students for 10 or more hours per week.

Volunteer Coaches

All volunteer coaches must complete the following clearances, forms and training requirements before being considered for a coaching opportunity.

ACT 151 – Pennsylvania Child Abuse Clearance for Volunteers

Apply online at https://www.compass.state.pa.us/cwis/public/home

Fee: Free to volunteers once within a five-year period.

• Log in or create an individual account through the Child Welfare Portal.
• Create a Keystone ID username and password.
• Store your Keystone ID and password in a secure place for future use.
• You will receive an email notification when your results are available.
• Print or save your results as a PDF.
• Submit your results to the Athletic Director.

ACT 34 – Pennsylvania State Police Criminal Record Check for Volunteers

Fee: Free to volunteers once within a five-year period.

Browser Notice: Do not use Safari when requesting a PATCH check. Safari is not supported by the PATCH system, and the request may not be completed.

1. Select New Record Check – Volunteers Only.
2. Review the Volunteer Acknowledgement section and select Accept.
3. Identify Moon Area School District as the organization for which you will be volunteering.
4. Enter the district phone number: 412-264-9440.
5. Follow the remaining instructions.
6. Print or save your results as a PDF.
7. Submit your results to the Athletic Director.

2. Select Help at the top of the page.
3. Open the drop-down menu.
4. Select the appropriate form to review and print.

ACT 114 – FBI Clearance or Volunteer Affidavit

Pennsylvania Residents for 10 or More Consecutive Years

Individuals who have lived in Pennsylvania for 10 or more consecutive years may submit the district-approved Volunteer Clearance Affidavit instead of completing FBI fingerprinting.

• Only the version approved by the Moon Area School Board may be accepted.
• Submit the completed affidavit to the Athletic Director.

Pennsylvania Residents for Fewer Than 10 Consecutive Years

Individuals who have not lived in Pennsylvania for 10 or more consecutive years must complete FBI fingerprinting.

Fee: $23.25, payable by credit or debit card.

To register:

1. Visit https://uenroll.identogo.com/
2. Pre-enroll online or call 1-844-321-2101 and select Option 2.
3. Enter the service code 1KG6XN
4. Confirm that the correct service code is entered before completing registration.
5. Select a fingerprinting location and schedule an appointment.

Fingerprinting may also be available through Beaver Valley Intermediate Unit 27.

Important: The email link used to access fingerprint results may only be opened once. Open the link on a device that allows you to immediately download, print or save the results.

Results are not automatically sent to the school district. Applicants must provide the district with the UEID number shown on the receipt issued after fingerprinting.

ACT 126 – Mandated Reporter Training

Moon Area School District requires all volunteer coaches to complete the free Recognizing and Reporting Child Abuse: Mandated and Permissive Reporting Training.

• The training takes approximately three hours.
• The course may be completed over multiple sessions.
• Log in or select Don’t Have an Account.
• Submit the required information and check your email for a username and password.
• Log in and select Access Your Course.
• Select the link to launch the course.
• Disable your pop-up blocker if the course does not open in a new window.
• Complete the course and print your certificate.
• Submit your certificate to the Athletic Director.

Pennsylvania Department of Education Required Coaching Education

All paid and volunteer coaches must complete both required courses annually, on or after July 1, and before participating in practices or other coaching activities with students.

After completing each course:

• Download the certificate of completion.
